If you would like more specific information on the Kindermusik Program at MACSA, please fill out the form below including at least the child's Date of Birth, parent/caregiver's first name, and your preferred method of contact (mail, phone, email, or any combination thereof). Ms. Jen will receive this information and respond as quickly as possible (usually within a few hours by email and within 24 hours by phone).  

If you are ready to register for a class, ALL the information below is required. You may fill out and submit the form below, or click here for a downloadable form to mail or bring in to MACSA. Either way, Ms. Jen will contact you within 48 hours to confirm receipt of your registration and make payment arrangements if necessary.
